Job Summary:

Upstate University Medical Associates at Syracuse Inc. is searching for an Ambulatory Quality Coordinator, who will report up through the Director of Quality for and is integrated into the Primary Care Clinics.

The Quality Coordinator performs processes necessary for optimizing quality programs for the organization including AQA Vizient, payor quality programs, PCMH & Departmental quality initiatives. One will provide patient outreach and quality metric support for providers and will be responsible for validating quality metrics as well as maintaining patient lists, monitoring and acting on gaps in care reports, patient outreach, patient scheduling and effectively communicating with members of the patient care team, including the patient.

Key Responsibilities:

  • Reconciling patient lists to include the patient lists associated with the measure numerator and denominators for appropriate and expected inclusion in domain and quality metrics.
  • Be an expert and a resource related to the methodology requirements for the AQA Vizient reports and the NCQA HEDIS Quality Metrics.
  • Send monthly reports to respective Primary Care and specialty clinics with measure progress updates.
  • Identify patients in the reports who may benefit from clinic-level outreach and work with the appropriate clinic team to address this need.
  • Provide education in the form of written and oral communication related to the quality metrics of interest to the impacted specialties and or departments.
  • Validation of current data mappings between the various utilized systems for data aggregation and maintenance of mappings once validation is complete to ensure any additions or deletions are captured appropriately.
  • Monthly validation of visit counts for AQA Vizient utilizing existing validation reports to ensure the scheduling database is accurate.
  • Review AQA Vizient Quarterly Calculators when they are released to verify visit volume, confirm data mapping is accurate, and confirm locations included are accurate.
  • Perform root cause analysis for low measure scores under the discretion of the Ambulatory Quality Governance Committee and report back to the committee with findings for opportunity and or barrier identification.
  • When feasible, align measure improvement initiatives with other department or specialty quality initiatives (e.g.  PCMH, VBP, Episodic Care, departmental quality initiatives).
  • Attend applicable committee, department, and faculty meetings to share quality improvement needs, strategies, and ideas.

Minimum Qualifications:

Bachelors Degree or an Associates Degree with 2 years of experience in a related field.

Must have strong analytical and problem-solving skills.

Be capable of independent work with the production of high-quality output.

Strong contributor to team environments, and be able to work effectively with people.

Preferred Qualifications:

Ambulatory office experience in the primary care setting or experience as a community health worker.

Experience using an Electronic Medical Record.

Work Days:

Monday - Friday, 8:00 - 4:30 pm
