Summary : Urology call coverage for Level II Trauma center and Hospital

Rate Caps for Submission :
Malpractice Must be included in rates.
Orientation xx200 half the hourly / callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Holiday Must be 1.5x the regular rate. Per holiday hour no OT
On Call (Daily) xx3700 and below Per 24 hours to include 4 hours gratis
24 Hour Holiday Call Rate xx5550 Per 24 hours to include 4 hours gratis
Call Back $400 and below Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Holiday Call Back $600 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

- April 25 @ 5p through April 28 @ 7a
- May 23 @ 5p through May 27 @ 7a
- June 06 @ 5p through June 09 @ 7a

Duties : Cover Urology call get handoff from Urologists if appropriate
Required Procedures : General Urology
EMR : EPIC
Support Staff : Surgery nurses and techs. on call
Reason for Coverage : previous locum scheduled cancelled stint
Deal breakers (do not present this) : Not willing to learn EPIC
Travel Preferences :

Air Travel : Local preferred but not required
Credentialing Information :
Timeframe : 34 weeks minimum
Requirements : M.D. or D.O.; DEA; NE license; case logs from last 2 years
Credentialing with other facilities No
Billing enrollment Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less : No they will need to be enrolled with our payors so that we can bill under their own name.

On average how busy is the call volume per day (Consults vs. callbacks) : Will have days with no consults for urology out of the ED. However there are other days where there may be 4 or 5
Is the Urologist responsible for admissions or will the hospitalists be covering admits Hospitalists admit patients
