Overview:
We are seeking an experienced MDS Coordinator to join our healthcare team. The ideal candidate will have a strong background in patient care and a deep understanding of medical office operations.
Pay and Shift Details: $3100/week for 40 hours & Day shift - 8:00am - 4:30 pm
Responsibilities:
- Must be familiar with South Dakota rules and matrix.
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents
- Ensure accurate and timely submission of MDS assessments to regulatory agencies
- Collaborate with healthcare providers to develop individualized care plans based on MDS assessments
- Utilize electronic health record systems such as eClinicalWorks, Athenahealth, or similar platforms
- Conduct comprehensive assessments of patients' health status and needs
- Assist in discharge planning and coordination of care transitions
- Maintain knowledge of HEDIS measures and ensure compliance with quality standards
- Work closely with managed care organizations to optimize reimbursement for services provided
Skills
- Proficiency in utilizing electronic health record systems like eClinicalWorks, Athenahealth, or similar platforms
- Experience in home care, primary care, or managed care settings
- Strong understanding of medical terminology and physiology
- Knowledge of discharge planning processes and best practices
- Familiarity with MDS assessment tools and requirements
- Excellent communication and interpersonal skills for effective collaboration with healthcare providers
