What We’re Looking For:

SuperCare Health is seeking a dynamic and results-driven Telecare Manager to lead the execution of our Key Focus Product (KFP) Program Management, oversee the patient Sleep Journey, and drive impactful Population Health programs. The ideal candidate will play a pivotal role in improving patient outcomes, ensuring program excellence, and collaborating across departments to meet business objectives.

What You’ll Do:

Key Focus Product (KFP) Program Management

  • Support in the design & development of Clinical KFP programs and initiatives
  • Lead the implementation and oversight of Clinical KFP programs & initiatives to align with organizational goals and patient outcomes.
  • Develop and maintain detailed project plans, timelines, and metrics to ensure the success of KFP programs.
  • Monitor program performance, identify opportunities for improvement, and drive data-driven decision-making.
  • Generate and present monthly KPI reports related to patient and program outcomes
  • Collaborate with cross-functional teams to ensure seamless program execution and scalability.

Oversight of the Patient Sleep Journey

  • Oversee the end-to-end patient Sleep journey, ensuring adherence to clinical best practices and operational efficiency through virtual training and RDT scheduling and integrated technology.
  • Enhance patient engagement through education, coaching, and support, ensuring optimal therapy adherence and outcomes.
  • Generate and present monthly adherence outcomes and other key metrics associated with PAP LOB
  • Collaborate with outsourced services and vendors partners to ensure proper outcomes and program efficiency
  • Work with Customer Service and Field Operations to address patient needs promptly, reducing barriers to care.
  • Utilize technology and telehealth solutions to improve the patient experience and streamline care coordination.

Population Health Programs

  • Manage population health initiatives that target high-risk respiratory and sleep-disorder populations.
  • Train, mentor, and coach staff on proper implementation of program protocols and procedures
  • Engage in patient enrollment and clinical assessments related to programs
  • Contribute to ongoing relationship building and management for payors and partners participating in SCH programs
  • Leverage data analytics to identify trends, manage care gaps, and implement targeted interventions.
  • Partner with Sales Leadership and Customer Service to align population health programs with business growth and client satisfaction.
  • Support scalable solutions that enhance the overall health outcomes of SuperCare Health’s patient base.

Collaboration and Leadership

  • Oversee and manage weekly/monthly/quarterly interdisciplinary meetings with payors, medical directors, and other participants of SCH programs
  • Collaborate closely with Field Operations, Corporate Operations, Sales Leadership, and other stakeholders to ensure alignment on strategic objectives.
  • Provide regular updates to the Director of Respiratory Services on program performance, challenges, and opportunities.
  • Work closely with the Clinical Operations Manager on creation and updating of Clinical training material for SCH including video and resource trainings for both internal and external use
  • Perform routine audits of program metrics, processes, and clinical interactions
  • Foster a culture of innovation, collaboration, and excellence within the Telecare team.
  • Train and mentor team members to ensure consistent delivery of high-quality care and service.

Other Duties:

  • Perform other related duties as assigned.

This role may be a fit if you have..

  • Valid RCP and RRT License
  • Bachelor’s degree in Healthcare Administration, Respiratory Therapy, Business, or a related field and/or combination of relevant education and experience.
  • 5 years of experience in program management, population health, or telehealth within the healthcare industry.
  • Proven expertise in respiratory care and/or sleep therapy programs is highly desirable.
  • Strong analytical skills with the ability to interpret and act on complex data sets.
  • Exceptional communication, leadership, and interpersonal skills.
  • Proficiency with healthcare technologies, telehealth platforms, and data analytics tools.
