About the Role
As a Senior LTSS Service Care Manager RN at Superior HealthPlan, you will be responsible for leading a multidisciplinary team to deliver high-quality care to our members. You will work closely with healthcare providers, community resources, and caregivers to develop and implement individualized care plans that meet the unique needs of each member.
Your responsibilities will include evaluating service needs, developing and implementing care plans, coordinating care delivery, and educating members and their families/caregivers on available services and benefit options.

Qualifications
To be considered for this role, you will need:
- A graduate degree from an accredited school of nursing or a Bachelor's degree and 4–6 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
