Key Responsibilities:
- Psychosocial Assessments: Conduct thorough psychosocial assessments of patients and families to identify emotional, psychological, and social needs.
- Emotional Support: Provide counseling, active listening, and therapeutic interventions to help patients cope with anxiety, depression, grief, and other emotional concerns.
- Care Planning and Advocacy: Collaborate with the interdisciplinary team to develop care plans that prioritize patient comfort, dignity, and quality of life.
- Crisis Intervention: Offer crisis intervention and immediate support when difficult situations arise, helping patients and families navigate challenging moments.
- Education and Resource Referral: Educate families about available resources, advance care planning, and assist with end-of-life decision-making.
- Documentation and Reporting: Maintain accurate records of all assessments and interventions, ensuring compliance with regulatory standards.
Qualifications:
- Master’s Degree in Social Work (MSW) from an accredited program.
- State Licensure: Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in the relevant jurisdiction.
- Experience: Experience in clinical social work, preferably in hospice or palliative care.
- Communication Skills: Excellent verbal and written communication skills to effectively engage with patients, families, and the care team.
