Responsibilities:
- Holistic Care: Provide comprehensive nursing care to hospice patients, focusing on physical, emotional, and psychological well-being.
- Care Planning: Develop and implement individualized care plans that address patient needs, preferences, and goals, ensuring comfort and quality of life.
- Family Support: Offer emotional support and education to patients' families, empowering them to navigate the end-of-life process with understanding and grace.
- Symptom Management: Monitor and manage symptoms effectively, collaborating with interdisciplinary teams to optimize pain and symptom control.
- Communication: Foster open and empathetic communication with patients, families, and the care team, ensuring everyone's concerns are addressed.
- Documentation: Maintain accurate and detailed patient records, documenting care plans, interventions, and outcomes according to hospice regulations.
- End-of-Life Planning: Assist patients and families in making informed decisions about advance directives, hospice care options, and end-of-life wishes.
Qualifications:
- RN Licensure: Active registered nurse license in the Minnesota, demonstrating competency and compliance with nursing standards.
- Hospice Expertise: Extensive knowledge of hospice principles, pain management, and symptom control to provide comprehensive end-of-life care is required.
- Compassionate Nature: Genuine passion for ensuring patients and families experience comfort and dignity during the end-of-life process.
- Interdisciplinary Collaboration: Proven ability to collaborate with a diverse team of healthcare professionals to deliver integrated patient-centered care.
- Effective Communication: Exceptional communication skills, both verbal and written, to facilitate meaningful conversations with patients and families.
- Critical Thinking: Strong problem-solving skills, enabling quick and effective decision-making in complex and emotional situations.
- Cultural Sensitivity: Respect for diverse cultural, religious, and ethnic backgrounds, ensuring personalized care that aligns with individual beliefs.
