Job Title: Hospice RN Case Manager
Pay: $90,000 – $130,000 DOE

Responsibilities:
- Holistic Care: Deliver comprehensive nursing care focused on physical, emotional, and psychological well-being
- Care Planning: Create and implement individualized care plans tailored to patient needs and goals
- Family Support: Educate and support families, helping them navigate the end-of-life journey with confidence and compassion
- Symptom Management: Collaborate with interdisciplinary teams to manage pain and symptoms effectively
- Communication: Maintain open, empathetic communication with patients, families, and team members
- Documentation: Accurately document all care provided, in compliance with hospice regulations
- End-of-Life Planning: Guide patients and families in understanding and planning for hospice care options and advance directives
Qualifications:
- RN Licensure: Active RN license in Minnesota
- Hospice Expertise: Strong understanding of hospice principles, pain management, and symptom control
- Compassionate Nature: A passion for supporting patients and families with dignity and empathy
- Interdisciplinary Collaboration: Experience working within a team-based model of care
- Effective Communication: Clear, compassionate verbal and written communication skills
- Critical Thinking: Ability to make thoughtful, informed decisions in high-pressure situations
- Cultural Sensitivity: Respect for diverse cultural, religious, and personal values
