Highland Chateau Health and Rehabilitation Center is looking for a Full-Time Social Service Coordinator in the Ramsey County area to become a part of our team!
Role Overview:
As a Social Service Coordinator, you will play a crucial role in providing comprehensive social services to our residents and their families.
Duties include but are not limited to:
Assessing the social and emotional needs of residents and creating individualized care plans.
Collaborating with interdisciplinary teams to develop and implement personalized care strategies.
Coordinating programs and activities for residents.
Providing counseling and emotional support to residents and their families.
Assisting with discharge planning and facilitating transitions to other care settings, if necessary.
Maintaining accurate and confidential records of resident interactions and progress.
Qualifications:
Bachelor’s degree in social work or a related field
Psychology or Social Work experience preferred
Previous experience in a long-term care or healthcare setting is highly advantageous.
Excellent communication and interpersonal skills.
Ability to work effectively as part of a team and independently.
