Qualifications
· Education: Graduate of a Social Work accredited program (BSW or MSW)
· Licensure: Licensed as a Social Worker in the State of Minnesota
· Experience: Minimum of two years of experience as a social worker, strong preference for previous hospice or palliative care experience.
· Skills: Ability to work independently and seek consultation and support as appropriate, skilled in crisis intervention and response, experience with community-based resources and referral process, advance care planning facilitation, familiarity with grief support, acceptance of different social, racial, cultural, and religious backgrounds, flexibility, innovation, and strong interpersonal skills, ability to cope or seek support for work-related mental and emotional stress, and ability to value all members of the interdisciplinary team.
· Transportation: must have a current, valid driver’s license with auto insurance and reliable transportation.
Position Details
- Monday-Friday with compensated on-call rotation schedule.
