Job Summary
A Support Specialist works with individuals with disabilities in the persons home, at a base site or in the community. They will provide assistance or training in the areas that are identified by each persons team. These areas may include but are not limited to : community access, supervision, household, employment, personal care, and / or money management

What you"ll do
Essential Job Duties
- Work with assigned individuals according to weekly schedule.
- Teach / role model the skills as identified by the IP (Individualized Plan). This could include : Community Access, Supervision, Household, Employment, Person Care, Money Management, or other skills need to live independently in the community.
- Support the person to develop connections / relationships in their local community.
- Support and encourage the person in making responsible choices.
- Document daily contact notes, ISSP (Individualized Support & Service Plan) Data Tracking Log / s, incident reports, and assist / complete other assessments or paperwork for the person.
- Notify your supervisor of any problems or concerns regarding the person.
- Attend and successfully complete all required trainings and attend departmental meetings to increase knowledge of programs and people served. Ensure all trainings remain current.
- Assist people served with medical / personal care needs including but not limited to changing / restroom assistance, G-tube feedings, and transferring.
- Transport people served via company bus, vans and personal vehicle.
- Bending, reaching, twisting, squatting are required when assisting people served with repositioning, transferring and other activities of daily living.
- Lift a minimum of 50 pounds; push / pull a minimum of 200 pounds.
Other Duties
Attend team meetings as needed for the person.
