Summary

As a Residential Service Coordinator at Support Solutions Inc., you will play a vital role in directly overseeing the daily operations of our Residential and Shared Living Programs, as well as the Residential Team Lead DSPs. This position is crucial in ensuring that individuals receive the highest quality of care tailored to their unique needs, aligning with our mission to inspire a culture of possibilities. The Residential Service Coordinator ensures that services are delivered in accordance with individualized care plans, promotes a safe and supportive environment, and fosters a culture of dignity, respect, and empowerment.

Responsibilities and Essential Duties

  • Implements and adheres to all policies and procedures of Support Solutions.
  • Interviewing and training employees; planning, assigning, and directing work; appraising performance; rewarding disciplining employees addressing complaints and resolving problems
  • Works collaboratively with appropriate state departments, agencies and families to provide quality services to each individual.
  • Completes, reviews, approves Therap accuracy daily for proper billing.
  • Provides on-call support to programs 24/7 Monday 8 am through Friday 4 pm; provides on call support on a rotating basis from Friday 4 pm to Monday 8 am.
  • Responsible for the development and implementation of program components, specifically focusing on each individual served in a person-centered approach.
  • Oversight and weekly submission of petty cash receipts.
  • Scheduling staff to meet the needs of the program and ensures that shifts are covered.
  • Direct support of clients supported in the absence of a Direct Support Professional.
  • Provide coaching and feedback, and completes staff performance evaluations.
  • Disciplines staff members as needed.
  • Audits and approves staff hours, mileage and documentation.
  • Responsible for all aspects of the residential home operations under their supervision
  • Responsible for compliance with state and federal regulations
  • Responsible for the quality assurance of services provided including assisting homes to develop a plan of correction to quality audits in the home to correct deficiencies
  • Conducts scheduled and unscheduled visits to the homes under their supervision (at least one visit weekly to each home). Provides report within 24 hours to Director of Care Management of findings
  • Ensures staff in the homes are following the Person Centered Plan for each individual served in home. Ensures Team Leads and Direct Support Professionals are incorporating community activities based upon individuals’ abilities and interests.
  • Responsible to seek and secure referrals
  • Maintains professional relationships and communication with team members, stakeholder’s and persons supported
  • Attends and participates in meetings, seminars, trainings and pertinent support groups that reflect the needs, concerns, and services of those supported by agency
  • Participates as part of a team, recognizing and appreciating the abilities, enthusiasm and beliefs of each member
  • Participates in the interviewing, hiring and training of personnel
  • Maintains a tracking mechanism for all PCPs (person centered plans) and ISPs (individual service plans).
  • Anticipates upcoming deadlines.
  • Coordinates meetings with service recipients.
  • Conducts service planning with service recipients, which may include other constituents. Service planning entails a review of old goals as well as an inquiry into the service recipient’s needs, unmet needs, desires and dreams.
  • Gains/researches information from team lead/staff/family members, psychological evaluations and other resources in order to develop Positive Support Plans and Functional Assessments.
  • Completes required forms in Evergreen.
  • Prints copy of Evergreen forms in preparation for PCP meeting. Scans copies to electronic file.
  • Notifies Director of Care Management that Evergreen entry is complete.
  • Provides courtesy notification to case manager that Evergreen entry for home supports is complete.
  • Once the effective plan date has passed, follow up with case manager to request a signed Personal Plan Face Sheet, denoting approval of the plan. This step is not necessary for those case managers that automatically distribute copies of the signed Face Sheet.
  • Prints completed PCP/SIP from Evergreen, and provides approved copy (with the signed Face Sheet) to team leader to file with a staff signature sheet.
  • Scans approved PCP to electronic file.
  • Maintains tracking about critical steps in the PCP process, for instance, Evergreen entry completion date.
  • Coordinates ISP meetings. Write/revise ISPs goals.
  • Writes ISP/ SIP narrative.
  • Gains service recipient or guardian, if applicable, approval on ISP plans.
  • Notifies Director of Care Management that ISPs are complete
  • Attends/documents pre-placement, post-placement, semi-annual, quarterly, special and Individual Support Team meetings as applicable.
  • Performs other duties as assigned by supervisor.

Education and Experience Requirements

  • Required: High School Diploma and/or GED/HiSet
  • Required: Bachelor's Degree (B.A) from four-year College or University; or one to two years related experience and/or training; or equivalent combination of education and experience.
  • Required: Valid Driver's License and Auto Insurance
  • Experience in home community care or working with individuals with developmental disabilities is essential.
  • Excellent communication skills for effective interaction with residents, families, and staff.
