As a Primary Support Professional, you'll work directly in our clients' homes, coordinating daily activities and ensuring the highest quality of care. You'll collaborate with Program Coordinators to implement personalized support plans, manage documentation, and maintain a safe, healthy living environment. Key Responsibilities:
- Coordinate and implement Individual Support Plans (ISPs) for assigned clients in their homes
- Train and oversee staff on clients' medical needs, medications, personal preferences, and compliance requirements
- Ensure quality care by maintaining detailed documentation, including daily notes, goals, and health tracking
- Schedule and accompany clients to medical appointments
- Maintain safe, clean, and healthy living environments, including proper food storage and safety equipment
- Manage client funds, ensuring accurate record-keeping and proper documentation
- Collaborate with Program Coordinators on quality management initiatives and follow-up care
- Coordinate staff schedules and provide shift coverage as needed
- Conduct and document required safety drills
- Monitor and manage home mileage budgets
- Participate in support plan meetings and service monitoring visits
Education and/or Experience Qualifications:
- High school diploma or GED required
- One year experience in intellectual/developmental disabilities field OR successful completion of Missouri Quality Outcomes training (approved by Division of DD regional office)
Additional Qualifications:
- Must be 18 years of age
- Current driver's license with acceptable driving record and current auto insurance required
- Strong communication and interpersonal skills
- Ability to read and interpret documents such as safety rules and procedure manuals
- Basic math skills and ability to maintain accurate records
- Must have reliable means of communication
- Background check clearance must be obtained, including criminal record, driving record, abuse/neglect, and fingerprint check
- Must complete New Hire Orientation at the beginning of employment
- Must complete all training requirements including Relias at the beginning of employment and annually thereafter
