As a Program Coordinator, you will be responsible for the development and implementation of Individual Support Plans (ISPs), ensuring that outcomes are clear and measurable. You will work closely with team members to maintain compliance with policies and procedures while promoting a culture of wellness and community participation.

Key Responsibilities:
- Coordinate and participate in the development of the Individual Support Plan (ISP); ensure outcomes are measurable and clear to team members.
- Supervise and evaluate team members, providing leadership and guidance to all aspects of the department.
- Conduct quarterly Record and Therapy Reviews to ensure records are current and complete.
- Make weekly visits to the ISL to monitor the status of the individual and ensure a safe and clean environment.
- Maintain client profiles in the database and prepare annual ISL budgets.
- Work in conjunction with Primary Support Staff and Residential Nurse to ensure medical needs are met.
- Complete daily documentation audits in Therap, including health, medications, and daily notes.
- Facilitate opportunities for natural support and community participation.
- Other duties as assigned.
Education, Experience, and/or Credential Qualifications:
- A bachelor’s degree from an accredited college or university.
- Relevant experience in the field of Developmental Disabilities can be substituted year for year for a degree.
Additional Qualifications:
- Must have a reliable means of communication.
- Must possess skills to read and understand written materials.
- Must possess skills to write clearly and legibly.
- May be required to assist with physical transfers or lifting, depending on the needed supports of the individual.
Physical Requirements:
- Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.
- Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
- Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
