What are the responsibilities and job description for the Care Coordinator (Healthy Start) position at Supporting Healthy Initiatives Tulsa (SHIFT)?
We are seeking a dedicated Care Coordinator to join our team.
Overview: SHIFT Healthy Start is designed to meet the needs of mothers, infants, and families living in Tulsa County, focusing on social determinants of health to reduce infant mortality. As a Healthy Start Program Coordinator, you will work with expectant mothers and their families before, during and after pregnancy employing strategies to improve maternal child outcomes.
In this role, you are primarily responsible for providing case management, comprehensive care, education, and resources to optimize maternal and child well-being. The Healthy Start Care Coordinator will work to provide maternal and child health services to Healthy Start clients, their significant other, partner and/or support system. In this role, you will make home visits, as needed.
What You Will Do:
- Assess pregnant individuals and families to identify their needs and risk factors.
- Develop individualized care plans based on assessment findings.
- Provide case management, activities and planned events for Healthy Start families.
- Work to eliminate barriers to accessing healthcare and social services.
- Provide education on prenatal and postnatal care, nutrition, childbirth, and infant care.
- Offer counseling and emotional support to clients facing challenges related to pregnancy and parenting.
Position Qualifications:
- Bachelor’s degree in social work, nursing, public health or related field, or an equivalent combination of education and related work experience
- Minimum of one year experience working in case management
- Valid U.S. driver’s license, insurance, and use of personal vehicle
- Strong written and oral communication skills
- Strong organizational skills
- Ability to work collaboratively
