Role Responsibilities:
Rounding on a daily basis, split between half the day in an acute hospital and half the day in a long-term acute hospital.
Caring primarily for geriatric patients, with some adult patients, supporting them and their families through difficult times.
Supporting hospital staff, attending physicians, and ensuring open communication lines among everyone involved in patient care.
Requirements:
Active Family Nurse Practitioner license.
At least two years of hospital inpatient experience and two years of primary care experience.
Ability to work independently with a patient-centered approach.
Excellent communication skills, with a compassionate bedside manner that fosters trust and respect with patients and their families.
Training and Development:
New hires will shadow the current team for several weeks, going through our processes until they feel comfortable. They will transition to independent work under a board-certified palliative physician.
Team and Support Structure:
Reporting to their supervising physician and the chief clinical officer, with occasional reports to the chief medical officer.
Access to support staff at each hospital, including social workers, chaplains, and patient advocates.
Additional support from our office staff for documentation, medical records, calls, and obtaining records.
