Job Summary
A Rewarding position as a Family Networks Service Coordinator.
Bachelor of Science or Degree in Human Services and/ or Related Field Required.
Good communication and organizational skills. The position is hybird model remote three days in-person office and two days remote.
Ability to conduct family conferences , case coordination , case management and assessment skills
Ability to effectively collaborative with service providers, DCF and other community supports.
Some driving required and possesses a valid driver's license.
The Family Networks Program is a joint partnership between Supportive Care and the Key Program
The Service Coordinator Position is through Supportive Care.
The Supportive Care's Service Coordinator is a professional who actively supports the philosophy of Supportive Care.
He/She is supervised by the Family Networks Program Director on an daily basis as well as direct accountability to Supportive Care;s Executive Director.
Responsibilities and Duties
The Service Coordinator is responsible for the coordination and case management of services between families and providers to ensure families success within the community.
Ability to conduct family conferences, case coordination, case management and assessment skills.
Ability to effectively work with service providers, DCF and other community providers.
Good communication skills and organizational skills
Attends case conferences, clinical meeting and educational meeting for families as needed.

Qualifications and Skills
Bachelor Degree in Human Services and or an Related Field.
Two Years clinical and or direct care experience in Human Services
Experience as a Service Coordinator working with families involved with DCF a plus.
