Supportive Care is a behavioral healthcare company with an extraordinary team of clinicians providing a full range of pharmacological, psychological, and behavioral health services to skilled nursing facilities, assisted living facilities, and memory care communities. This position is for 20-30 patients per week (1 Day Caseload).
Responsibilities :
- Establish open lines of communication for individuals with mental or emotional issues.
- Perform Psychological Evaluations and Maintain Weekly, Bi-Weekly, or Monthly F / U Sessions as needed
- Medication Management
- Maintain accurate documentation.
- Implement therapy plans that fit each patient's personal needs.
