ORGANIZATION
The mission of Supportive Housing Communities (SHC) is to provide affordable housing to alleviate homelessness and human suffering. To carry out this mission, SHC prioritizes the needs of the chronically homeless—those who have been homeless for at least a year and are living with one or more disabling conditions. The organization uses a permanent supportive housing approach, which links permanent, affordable rental housing with access to flexible, voluntary supportive services (case management, mental health/substance use treatment and employment services) to help this most vulnerable population maintain stable housing and live independently in the community.
POSITION AND RESPONSIBILITIES
The Case Manager, a position reporting to a Program Coordinator, is responsible for providing intensive case management, clinical and support services to individuals and/or families who are experiencing homelessness or at risk of homelessness. The Case Manager assists individuals and families in developing skills and supports necessary to maintain housing and achievement of self-identified goals; provides clinical assessments and individual psychotherapy counseling; assesses the individual and families’ needs and works with them to identify appropriate goals; monitors progress on goal attainment; and coordinates linkages and referrals to other community resources and services.
The primary responsibilities of the Case Manager include, but are not limited to, the following:
Case Management
- Establish a trusting relationship with the individuals or families participating in Permanent Supportive Housing programs to promote their well-being and housing stability;
- Provide case management services as appropriate to each individual or family’s acuity level and case specific needs;
- Coordinate services and monitor progress on individual or family referrals and linkages to community resources and other service providers;
- Conduct home visits, as required;
- Facilitate coordination of the integrated case plans with the child welfare system or adult protective services, if required;
- Provide or link clients to appropriate substance use and/or mental health treatment, as needed;
- Provide individual or families with referrals to medical services, as needed;
- Provide coordination of academic support, including meeting with Charlotte-Mecklenburg Schools’ social workers and other staff and providing support with Individual Education Plans. Monitor academic achievements, promotions to the next grade level and school attendance for children and youth;
- Assist individuals and families in establishing goals and assessing barriers to achieving goals; Establish outcome objectives for interventions and monitor progress towards goal achievement;
- Provide situation-specific skills training, as needed;
- Participate in individual or family case presentations, consultations and team meetings;
- Maintain thorough case management documentation according to HMIS and SHC policy;
- Performs other duties as assigned by the Program Coordinator and Director of Program Services;
- Uphold the Supportive Housing Communities Policies and Procedures and the Social Work Code of Ethics at all times.
EXPERIENCE AND EDUCATION
- Bachelor's degree in social work, human services or other related field and at least three years’ case management experience;
- Experience and knowledge of the North Carolina child welfare and protective services system a plus;
- Experience working with racially diverse populations and demonstrated cultural competency skills;
- Must have a valid driver’s license;
- Must be able to work independently and in a team setting;
- Must be able to communicate well with families and community service providers;
- Must be able to communicate effectively orally and in writing;
- Must have a working knowledge of community resources and conflict mediation;
- Must be able to intervene in and resolve crisis situations;
- Must be able to demonstrate good judgment, especially under pressure;
- Adherence to confidentiality laws;
- Ability to comply with policies, rules and procedures of Supportive Housing Communities.
COMPENSATION
The expected hiring range is $45,000-$49,000 with a full benefits package.
