MORE DEETS ON THE ROLE:
You'll be joining our recently launched St. Louis County case management team! You will be based out of our new office in Duluth, with some flexibility to work from home once ramped up in the role. You will be at the ground floor, helping to train new team members and shape the department as it grows! Some of the duties within the case manager role would include:

Provide case management services to people living with disabilities

Complete new client intakes

Develop initial service plans with the client and legal representative

Coordinate services between providers and clients

Schedule in-person client meetings with clients in their homes at least twice per year or based on need

Document a variety of aspects of the client's care including: Service Plans, progress on goals, CSSPs, Service Agreements, etc.

Attend weekly team meetings and statuses with your supervisor

Attend first week of onboarding training, as well as quarterly trainings, at our headquarters in St. Paul (Per diem and mileage reimbursement provided)
With each client, you will work to uncover their needs and aspirations, assisting to remove any barriers that get in the way. You'll be a liaison for your client to ensure that their whole care team is well aligned, each doing their part to support their goals and meet their needs. We provide services to clients on the following waivers/programs; CADI, BI, AC, EW and DD.
SCHEDULE & COMPENSATION:

Monday – Friday, daytime hours

Hourly Rate $22-$25, dependent on experience/education
-
$22 - very little-to-no experience in human services
-
$23 - Experienced waiver case manager or experienced human services professional

$25 - 5 years of experience as waiver case manager or 10 years as a human services professional

Hiring Bonus: $500 to be paid out after successful completion of introductory period, $500 to be paid out after six months of employment. Must be in good standing.

SOME IMPORTANT QUALIFICATIONS:

To be eligible to provide AC/BI/CADI/EW Case Management you must have:
-
Bachelor’s degree in health and human services OR bachelor’s degree in any field with 1-year of experience in the human services field

To be eligible to provide DD Case Management you must have:
-
Bachelor's degree in social work, Special Education, Psychology, Nursing, Human Services or other related fields AND
-
One year of direct experience working with individuals with developmental disabilities or related conditions OR have taken a course with a focus on developmental disabilities

Ability to pass DHS background check
-
Driver’s license & vehicle insurance
