Provide compassionate and supportive counseling to patients and families facing end-of-life care.
- Collaborate with interdisciplinary team members to develop and implement care plans.
- Conduct psychosocial assessments and provide appropriate interventions.
- Assist patients and families in accessing community resources and support services.
- Maintain accurate and timely documentation of patient interactions and progress.
- Ensure compliance with all regulatory requirements and ethical standards.
- Participate in team meetings and contribute to the overall care planning process.
- Demonstrate empathy, professionalism, and cultural sensitivity in all interactions.
Requirements : Active License
At least one year of hospice experience
