Job Title : Hospice RN Case Manager
Job Type : Full-time
Location : Carrollton Tx, Kaufman Tx, Canton Tx
We are seeking a compassionate and experienced Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will be responsible for providing end-of-life care to patients and their families. You will work closely with interdisciplinary teams to develop and implement care plans that meet the physical, emotional, and spiritual needs of patients.
Responsibilities :
- Conduct initial and ongoing assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with interdisciplinary teams
- Provide skilled nursing care to patients in their homes or in hospice facilities
- Administer medications and treatments as prescribed by physicians
- Educate patients and families on disease processes, symptom management, and end-of-life care
- Coordinate care with other healthcare providers and community resources
- Document patient care and progress in electronic medical records
- Provide emotional support to patients and families during the end-of-life process
- Participate in on-call rotation as needed
