Job Summary
This is a Full Time, Salaried position (40 hours/week) with a $35k starting salary - willing to adjust pay based on applicant's experience.
The Case Manager plays a crucial role in supporting individuals and families in navigating recovery from Substance Use Disorder. This position involves assessing client progress, assisting with life skill development, and coordinating services to ensure that clients receive the support they require. The Case Manager will work collaboratively with various colleagues, healthcare providers, the legal system (as needed), and clinical team at Shore Break Recovery.
Duties
- Provide Case Management for residents with a Substance Use Disorder at Surfside Recovery Services
- Communicate weekly progress report to families
- Offer additional communication (as needed) with local providers or support system based on the needs of the resident
- Monitor medications and drug testing protocol
- Act as Clinical Liaison to Shore Break Recovery's treatment team
- Assist with Resident in Phase Development (through use of resident handbook)
- Assist in setting up appointments and supporting in follow-through
- Complete intake process with new residents
- Responsible for supervising/accounting for residents at various times
- Provides transportation to/from appointments as necessary
- May include, based on hours work, assistance with Evening or AM in-home goal-setting groups; may occasionally provide overnight coverage for the house manager.
- Other duties as assigned by Program Director
Skills
- Bachelor's Degree (or enrollment in) Social Work, Psychology, or related field
- Preferred 2 years' experience working with individuals with Substance Use Disorder
- Evidence of addictions-related coursework
- Willingness to learn and follow ethical guidelines
- Ability/Desire to motivate those who may be less motivated
- Ability to speak up/be direct if someone is struggling or engaging in problematic behaviors
- Patience, tolerance and empathy for our residents and their families
- Ability to multi-task, use computers and follow schedules
- Valid Driver's License with clean driving record
