Licensed Social Worker
Full-Time (M-F)
Role/Position Definition:
Ensures that a quality social work program for the individual patient and family is developed and implemented while concurrently monitoring service utilization and length of stay appropriateness.
Qualifications/Position Requirements:
A. Education/Experience
• Bachelor’s degree in social work or human services from an accredited school required.
• 2 years of Social Work experience preferred.
• Previous experience in health care setting preferably in a Skilled Nursing / Short term rehabilitation facility.
B. Licensure/Certification
• A current unrestricted Social Work License from the State of Michigan required.
C. Knowledge, Skills and Abilities
• Ability to react effectively to the needs of patients and families.
• Must possess excellent written and verbal communication skills.
• Ability to exercise professional judgment and initiative when analyzing problems and recommending solutions.
• Knowledge of state and federal regulations required for Social Work departments in Nursing homes.
• Basic computer skills, including use of Microsoft Office software.
Work Environment:
A. Exposure to patient care areas.
B. May be exposed to blood and body fluids, sharp blunt or irregular surface objects, and hazardous chemicals.
Duties and Responsibilities:
A. Conducts evaluations to determine the patient’s discharge plan, discharge needs, emotional and social needs.
B. Encourage and enhance patient, family and other necessary agents in the involvement in the treatment and disposition of planning.
C. Facilitate post-dispositional adjustment of the patient and family.
D. Enhance understanding and adjustment to the patient’s disability by providing support, education and training.
E. Assist with arranging family meetings and post-discharge services.
F. Communicate with the clinical staff as needed for clarification of the patient’s status and update them on the discharge plan.
G. Provides documentation in a legible manner to maintain the quality of the medical record as it relates to reimbursement guidelines, accuracy and facility requirements.
H. Participate in team and family conferences as necessary.
I. Completes all MDS appropriate documentation and interviews.
J. Promotes current standards of clinical practice through participation in continuing education and in-service programs.
K. Participates in program development.
L. Provides in-services to other staff members on Social Work and patient rights issues as it relates to patient care.
M. Performs all other duties as assigned.
