Role/Position Definition: Responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. Involvement on the unit should include, but is not limited to meetings with the nurses to solidify a team concept and explain Medicare forms and the importance of proper documentation. Communication is necessary to ensure that information from all personnel is collected and used appropriately.
Qualifications/Position Requirements:
Education/Experience
- Graduate of accredited school of nursing required.
- 1 years of experience as a RN strongly preferred.
- 1 years of experience providing care in a hospital setting preferred.
- Experience in a Skilled Nursing Facility or long-term care setting required.
Licensure/Certification
- Current unrestricted RN License from the State of Michigan required.
- Current Basic Life Support Certification for Healthcare Providers, from the American Heart Association required.
Knowledge, Skills, and Abilities
- Knowledge of MDS assessment forms/processes and state/federal regulations.
- Knowledge of BLS protocols in code situations, medication interaction/reactions, pain management, wound care, and performance of patient assessments.
- Knowledge of medical terminology, human anatomy, and physiology.
- Strong knowledge and proficiency in computers usage, including MS Office.
- Requires proficiency in the use of general office equipment (i.e. copier, fax machine, and telephones).
- Requires frequent judgment and response; occasionally in response to disasters or emergencies, actual events or drills.
- Requires high-level visual and auditory acuity for timely response and completion of patient assessments and care plans.
- Requires effective listening and language skills adequate for high-level written and interpersonal communication.

Duties and Responsibilities:
- Completes assessments, Minimum Data Set (MDS) and care plans for all residents assigned.
- Assesses charts and communicates with health care teams to create applicable health care plans for current and incoming residents.
- Conducts care plan conferences for assigned residents.
- Identifies needs for innovations or improvements in resident care and may assist in planning and implementing improvements.
- Informs supervisor of incomplete and/or untimely assessments by disciplines other than nursing.
- Maintains current knowledge of MDS assessment process
- Maintains and updates all care plans and assessments for residents as needed.
- Follows appropriate universal precautions to ensure safety and wellbeing of self and others and to comply with governing body regulations.
- Promotes and participates in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current/ new regulations.
- Communicates regularly and effectively with multidisciplinary team in order to continuously improve the quality of services provided.
- Maintains knowledge of state and federal regulations.
- Maintains knowledge of the MDS assessment forms.
- Formulates and implements individual care plans for residents.
- Records must be kept regarding Medicare days available, days used, and days left that is easily accessible.
- Any condition change for the better or worse should prompt revising of the MDS, updating the care plan, and closely monitoring eligibility of Medicare coverage. These condition changes will be reported to the Director of Nursing as soon as they are discovered.
- Participates in facility management meetings as requested.
- Educates Patient Care Techs on ADLs scoring as needed.
- CPSI documentation is accurate, consistent, and concise.
- Performs all other duties as assigned.
