What are the responsibilities and job description for the Quality Specialist position at Surry Rural Health Center?
The Quality Specialist is responsible for supporting the development, implementation, and evaluation of quality improvement initiatives within the guidelines of the Accountable Care Organization. This role focuses on monitoring patient care quality, identifying opportunities for improvement, conducting data analysis, and assisting the facility to achieve quality care goals, reduce hospital readmissions, and enhance patient outcomes. The role works closely with the ACO Quality Nurse and the referrals team.
Responsibilities
- Participate in quality improvement projects and initiatives to meet regulatory and ACO performance requirements
- Identify gaps in care or opportunities for improvement and collaborate with other staff to address these issues
- Collect and analyze data related to patient care, outcomes, and quality indicators
- Track performance on ACO quality metrics
- Participate in regular meetings with ACO staff and leadership to discuss performance metrics, patient outcomes, and quality initiatives
- Work closely with physicians, nurse practitioners, care managers, and other healthcare professionals to enhance care delivery
- Assist with completing patient care gaps and closing the referral loop to meet quality measures
- Stay current on new healthcare regulations, quality standards, and industry trends to ensure best practices are followed
- Monitor patient outcomes and healthcare quality measures as defined by the ACO
- Schedule, confirm, and reschedule patient appointments based on quality needs
- Complete clerical duties including answering phones, responding to emails
- Pulls charts, records, and other medical information when needed to close care gaps
- Participate in quality improvement projects
- Handles other duties as assigned
Qualifications
- High school diploma or equivalent
- Knowledge of modern office equipment
- Ability to operate a personal computer
- Experience in ACO or managed care preferred
- Previous experience in a healthcare setting
- Understanding of medical terminology
- Excellent communication and organizational skills
