Communicates with client and/or family and meets patients needs for courtesy, information, safety, privacy, and confidentiality.2. Follow all infection control procedures and policies (including use of universal/standard precautions).3. Maintains specific areas of Home Health documentation system as assigned by Department Director.4. Provides assistance as necessary to complete safe personal care that includes bathing, dressing, grooming, hair care, nail care, oral hygiene, changing of bed linen, shaving, deodorant application, skin care with lotions or powders, foot care, ear care, feeding assistance, assistance with elimination (enemas, routine catheter care, routine colostomy care), assistance with ambulation, changing of position in bed and assistance with transfers.5. Provides simple dressing changes which do not require the skills of an RN, but under the supervision of an RN: observing and practicing all infection control/universal precautions.6. Assisting the client with medications that are ordinarily self-administered and do not require the skills of an RN or LPN, and under the guidelines of the Kansas Home Health Regulations; in a safe and effective manner.7. Assistance with activities that are directly supportive of skilled therapy services but do not require the skills of a therapist to be safely and effectively performed. Examples of such activities may be routine maintenance exercises, repetitive speech routines.8. Collects urine, stool, or sputum specimens from the client, observing and practicing all universal precautions, and delivering to the hospital laboratory as directed by the supervising RN. When delivering of biohazardous materials, observes the agency policy and procedure in safe transport of all materials. 9. Reports changes in the client condition, client needs and daily or caregiver situations to the supervising RN verbally and /or in writing in a timely manner.10. Attends staff meetings, in-services, case conference meetings so to maximize coordination of services to the client.11. Completes 12 hours of Home Health Aide in-service hours per year as required by Medicare and the Kansas Home Health Care Regulations.12. Provided a positive working environment for all co-workers, colleagues, and visitors to the department through two-way communication, good listening techniques and consideration of differing opinions or difficult situations.13. Provides effective communication with client and family/caregivers in the home in ways that improve client morale and contribute toward meeting goals of client discharge from program.14. Follows the written instructions on all client care per the Plan of Care (485), the aide assignment sheet and any superseding physician orders.15. Maintains adequate supplies for use during all client care and follows procedure for utilization of supplies and equipment. Completes weekly charge/mileage records per agency policy.16. Maintains confidentiality of all client information, as well as employee and departmental information.17. Performs other duties as assigned.Physical Demands:Requires frequent sitting, standing, walking, bending, and grasping. Requires frequent bending, twisting, and stooping. Will require occasional climbing, balancing, kneeling, and reaching, grasping, and overhead work. Occasionally requires lifting and carrying up to 55 lbs. May also require pushing and pulling up to 55lbs. Job requires full range of body motion, with occasional twisting, climbing, balancing, stooping, kneeling, crawling, and reaching. Job may require reaching overhead. Eye-hand coordination, and finger and manual dexterity are required. Requires corrected hearing and vision to a normal range. Job requires working under stressful conditions and/or irregular hours. Some exposure to communicable diseases, sharps, and hazardous material is anticipated. Requires the ability to distinguish letters and symbols. Use of office equipment such as telephones, computers, copying machines, and intercom may be required. Repetitive movements such as writing are required. Will be exposed to extreme heat and cold due to outdoor work and need to travel to/from clients home.Working Conditions: Includes working weekends and holidays, with potential for varied shifts, overtime, cost containment absent days, or standby time.This is a position working 40.00 Hours/Period8AM-4:30 PM
