Schedule Details: Shift: Days. M-F 8-4:30 Transitional Care Management is a specific role within the RNCC department, and TCM RNs need to be team players who collaborate within the department and across the care continuum. The TCM RN quickly builds rapport with patients over the phone, identifies and addresses concerns that may be a barrier to a smooth transition from inpatient discharge to the community setting. The nurse who can rapidly assess, prioritize, and adapt to fluctuating patient needs/call volume will thrive in this role.
