Organization: SEBMF-East Bay Medical Foundation Position Overview: Provides medical nutritional therapy (MNT) and Diabetes Self-Management Education (DSME) to patients with diabetes, pre-diabetes, disease processes related to cholesterol, renal, and endocrine disorders, but also leads the day-to-day activities of the other Clinical Health Educators. Works collaboratively with the treating physician to manage and monitor treatment and the response to therapy. Provides ongoing education and consultation to nursing and other ambulatory staff, supports and provides physician education, and supports clinicians in hospital and other patient care environments. Provides nutritional care (MNT) and counseling to adults and children on clinician referral, collaborates with the health care team. Job Description: EDUCATION: Bachelor's: of Nutrition or Dietetics or related field OR Other: Graduate of an accredited school of nursing CERTIFICATION & LICENSURE: RN-Registered Nurse of California or RD-Registered Dietitian AND CDCES-Certified Diabetes Care and Education Specialist BLS-Basic Life Support Healthcare Provider TYPICAL EXPERIENCE: 8 years recent relevant experience SKILLS AND KNOWLEDGE: Knowledge of all aspects and options in the care and management of diabetes is required. Understanding of the adult learning model and group dynamics is required. Ability to work with a team to develop collaborative educational approaches and curriculum. Must have clinical assessment skills that support autonomous practice in the ambulatory setting. Team leadership and organizational skills.
