Organization: SAFH-Sutter Auburn Faith Hospital Position Overview: This position facilitates the transfer of a patient from the acute/sub-acute, or community. settings to the home setting. This role is critical in assuring seamless coordination in the transition of care. This is done in cooperation with the attending physician and hospital case management staff and community representatives. This position is responsible to coordinate all pertinent home care services provided by the home health agency. This is accomplished by working closely with the patient and or family as well as the facility's discharge planning/case management staff, physicians, and nursing staff. This position assures the patient meets the qualifications for Homecare services. This position collects and validates the patient's insurance information as necessary to assess for appropriate placement in Home Care and Hospice services. This position works directly with the Intake Department of the Home Health Agency to facilitate the transfer to the home setting. This position also acts as a clinical resource for physicians, nursing and facility staff for appropriate home placement, medication reconciliation, problem solving, and identification of needed services. Job Description: EDUCATION: Graduate from an accredited school of nursing California Public Health Certificate or completion of Community Health Nursing program. CERTIFICATION & LICENSURE: DL-Valid Drivers License Class C AUTO-Automobile Insurance RN-Registered Nurse of California BLS-Basic Life Support Healthcare Provider TYPICAL EXPERIENCE: 2 years recent relevant experience SKILLS AND KNOWLEDGE: Possess critical thinking and communication skills, in-depth knowledge of home health and infusion related medical terminology. Should also possess knowledge of federal, state, managed care regulations and reimbursement methods. Demonstrate ability to work as a contributing team member. Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work. Must think critically to both effectively plan and organize department operations consistent with Sutter Care at Home’s strategic and operating objectives and to effectively solve unique problems as they arise or identify when to consult supervisor. Must be able to deal with challenging work environment with time demands and occasional conflicting priorities. Must maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements and is responsible for maintaining all required licensure and certifications.
