Job Summary
We are seeking a highly skilled Registered Nurse to serve as our MDS Coordinator. The ideal candidate will have a proven track record of success in sub-acute nursing and rehabilitation settings.
Key Responsibilities:
- Serve as a liaison between healthcare teams, administrators, and regulatory agencies
- Collaborate with nurses, therapists, and other healthcare professionals to ensure accurate and timely completion of Minimum Data Set (MDS) assessments
- Utilize expertise in MDS and PPS processes to optimize resident care and meet regulatory requirements
- Analyze data and trends to inform quality improvement initiatives
Requirements:
- NYS Licensed RN with a minimum of 5 years' experience
- Proven track record of success in sub-acute nursing and rehabilitation settings
- Excellent communication and interpersonal skills
- Able to work independently and as part of a collaborative team
- Proficient in computer software applications, including SIGMACARE experience preferred
