We are seeking a compassionate and dedicated Direct Support Professional to join our team. In this role, you will provide essential support and assistance to individuals with disabilities, helping them achieve their personal goals and enhance their quality of life. You will work closely with clients in various settings, including group homes, nursing facilities, and in-home care environments. Responsibilities
- Assist clients with daily living activities such as personal hygiene, meal preparation, and medication management.
- Provide companionship and emotional support to enhance clients' social interactions and overall well-being.
- Monitor clients’ health by observing vital signs and reporting any changes or concerns to the appropriate healthcare professionals.
- Implement individualized care plans tailored to each client's needs, ensuring adherence to established goals.
- Facilitate community engagement by encouraging participation in social activities and outings.
- Maintain accurate records of client progress, incidents, and any relevant observations during your shifts.
- Collaborate with a multidisciplinary team including nurses, social workers, and other caregivers to ensure comprehensive care for clients.
Experience
Ideal candidates will have experience in one or more of the following areas:
- Nursing home or hospice care settings
- Home health services providing direct support to individuals with disabilities
- Social work or caregiving roles that involve patient observation and assistance
- Experience working in group homes or similar environments
