Job Summary:
The Supported Living Coach provides individualized supports to adults with developmental disabilities who live independently in the community. The primary goal is to assist individuals in achieving and maintaining their personal goals related to independent living, community participation, self-advocacy, and health and safety.
Essential Duties and Responsibilities:
- Provide one-on-one coaching and support based on each person’s needs, preferences, and support plan.
- Assist individuals in managing daily living activities such as budgeting, meal planning, grocery shopping, hygiene, medication management, and housekeeping.
- Support individuals in accessing and participating in community activities of their choice.
- Educate individuals on rights, choices, self-advocacy, safety, abuse, neglect, and exploitation.
- Assist with scheduling and attending medical appointments and follow-up care.
- Encourage and support social interactions, peer relationships, and natural supports.
- Maintain accurate, timely documentation of services provided (e.g., daily notes, progress reports).
- Collaborate with family members, service coordinators, and other professionals to ensure quality care and support.
- Respond to crises and provide guidance as needed to ensure the health and safety of individuals.
- Participate in required training, meetings, and ongoing professional development.
Qualifications:
- High school diploma or equivalent required; college coursework in Human Services or related field preferred.
- At least one year of experience working with individuals with developmental disabilities or related populations.
- Valid driver's license, clean driving record, and reliable transportation.
- Ability to pass background screening per state and federal regulations.
- Completion of required trainings (e.g., Zero Tolerance, HIPAA, Direct Core Competencies, First Aid/CPR).
- Strong communication, organizational, and problem-solving skills.
- Compassionate, patient, and respectful approach to care and support.
Working Conditions:
- Work is performed in individuals’ homes and in the community.
- Flexible hours, may include evenings and weekends.
- May require lifting or physical assistance with certain daily living activities.
Job Type: Part-time
Pay: $20.00 - $22.00 per hour
