At UW Health in northern Illinois, you will have :
POSITION SUMMARY :
Plans, evaluates and administers Speech Therapy services and care to patients in the home as prescribed by referring physician on an intermittent basis in accordance with Agency policies. Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as Health System initiatives. Coordinates family involvement in assessment and treatment of the patient.
EDUCATION / TRAINING :
Master’s Degree in Speech Language pathology. Home Care experience preferred.
LICENSURE / CERTIFICATION :
Clinical competency Bi-annual and CPR certification. Current license from the State of Illinois or in the process of acquiring licensure. Valid State driver’s license and proof of liability insurance. An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
EXPERIENCE / SKILLS / ABILITIES :
Successful completion of annual clinical, age and job specific competencies and skill verification tools required. Must be skilled in a broad range of modalities with clear understanding of indications and contradictions for each. Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner. Knowledge of TJC, IDPH and CMS rules and regulations. Willingness to accept responsibility. Ability to direct technical, assistant staff and students and organize work assignments. Leadership skills essential to the position.
