Summary/Objective: Provides patient centered medical care and/or guidance for adult, geriatric, and pediatric patients in Sweeny Community Hospital Clinic, West Columbia Clinic and may include Sweeny Community Hospital addressing physical, cultural, and psychosocial domains.
Essential Functions:

- Performs physical exams, orders and interprets diagnostic tests to diagnose and treat acute and chronic diseases
- Works in conjunction with supervising physician assisting in the management of chronic illnesses
- Develop, educate, implement, and serve as consultant to clinic staff concerning disease management
- Educates and counsels patients and families regarding Preventive Health measures including vaccine administration recommendations
- Participates in the development, execution, and periodic review of the written policies governing the services the clinic furnishes
- Participates with the physician in a periodic review of the patients’ health record
- Assesses patients’ needs and coordinates services
- Arranges for or makes referrals to needed and appropriate specialty medical services that cannot be provided at the clinic
- Communicates decisions and collaborates with staff, providers and others participating in patients’ care
- Documents in accordance with established policies and protocols
- Facilitates patient/family care discussion/conferences
- Performs other therapeutic and corrective measures as indicated, including urgent care
- Assists in all facets of care coordination for referrals from and to other participating medical providers
- Prepares and maintains accurate patient records and charts, and documents to support sound medical practice and reimbursement for services provided
- Works collaboratively with other patient care leaders to establish policies, procedures, and protocols as well as provide training for affected staff
- Facilitates or assists with Advanced Care Planning considering patient values and preferences
- Provides Medical and Preventive Services in accordance with the clinic’s policies
- Assists in ancillary and clinical staff supervision to provide the services essential to the operation of the clinic
- May also provide orders and care plan oversight to patients being managed by Home Health Care Agencies, Hospice Agencies or in a Long Term Care setting.

Supervisory Responsibility:
This position has supportive supervisory responsibilities including the ancillary and clinical staff required to provide the services essential to the daily operation of the clinic.
Work Environment:
This job will require exposure to patients with acute and infectious illnesses. This role routinely uses physical assessment equipment including EKG machine and standard office equipment such as computers, phones, photocopiers, filing cabinets, fax machines, and other equipment as needed.

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This position requires continuous physical activity, endurance, ability to prioritize activities and adapt to changing demands. Duties regularly require standing, walking, bending, kneeling, stooping, pushing, pulling and crouching as well as lifting and/or moving up to 50 lbs.
Position Type/Expected Hours of Work
This is a full-time position, and core clinic hours are Monday through Friday 8:00 am – 5:00 pm. It is understood that each provider is called upon to work extended hours to accommodate the needs of our community. To balance this demand, each individual clinic provider may have a different day off every week. This position requires work schedule flexibility based on the needs of the facility. The West Columbia Clinic is open Monday through Thursday 8:00 am – 5:00 pm and Friday 8:00 am -12:00 pm.
TRAVEL
Occasional travel is expected for attendance to conferences for maintenance of clinical knowledge, certifications and continuing education for licensure.
Required Education, Experience and License
Master’s Degree
Ability to work with various age groups
Professional License and certification for Nurse Practitioner
Preferred Education, Experience and License
At least 3 year(s) of experience working with all age groups in Primary Care

DEA and DPS Licensure
