Position Overview:
The Emergency Department Case Manager is responsible for an initial medical review for admitted patients and transition planning. Other responsibilities include care coordination and identification of patient.

- Ability to interact with patients, families, physicians, co-workers and community agencies.

- Ability to organize and prioritize multiple requests and duties
- Ability to maintain high moral standards both at work and in the community
Duties and Responsibilities:
- Initial medical necessity review using an approved tool( MCG, InterQual) to help access the appropriate status (inpatient or observation).
- Collaborate with ED nurses and providers to create patients discharge plans. This may include assistance with setting up equipment and medication, home health referrals and post -acute facility transfers.
- Manage high-use patients. Identify patients that meet high user qualifications criteria and work on compressive post-discharge care plan that includes primary care coordination, insurance enrollment, transportation and housing arrangements.
- Address regulatory issues including but not limited to: assure medical necessity and level of care, CMS two midnight rule, CMS condition code 44, Medicare Outpatient Observation Notice (MOON), and Emergency Medical Treatment and Active Labor Act (EMTALA).
- Ensure timely, accurate, and complete documentation of clinical information.
- Collaborate with Case Management and Utilization Review on the transition of patients from the ED to Acute or Intensive care units.
