Minimum Qualifications:
Qualifications
include a current RN license in the State of Montana (BSN preferred), a valid
driver’s license, appropriate auto insurance, and reliable
transportation. 2 years of recent acute care, home care, or hospice
experience including such clinical skills as venipuncture, catheterization,
wound care and/or parenteral therapy preferred.
Job Summary:
The
RN – Case Manager provides skilled nursing, care, and case management for
assigned patients in their place of residence; admits patients onto services;
coordinates and integrates services to ensure continuity of care; facilitates
the achievement of quality, clinical and cost outcomes; negotiates, procures
and coordinates services and resources needed by the patient or family; creates
opportunities and systems to enhance outcomes; and uses the nursing process of
assessment, planning and implementation of care, including patient/family
instruction and evaluation through an inter-disciplinary team approach.
This
position may start as a Visiting RN and move to Case Manager. The same clinical
responsibilities apply to both designations. To move to Case Manager, the
incumbent must carry a half case load as described in the Collective Bargaining
Agreement with the Montana Nurses Association Local #32.
Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. In this role, you will provide high-quality nursing care to patients in a hospice setting, focusing on comfort and quality of life for individuals with terminal illnesses. You will work closely with patients, families, and interdisciplinary teams to ensure that care is tailored to meet the unique needs of each patient.
Duties
- Conduct thorough assessments of patients’ physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Provide support for various medical procedures including catheterization, tube feeding, phlebotomy, and spinal taps as needed.
- Offer memory care and behavioral health support to enhance patient comfort and well-being.
- Educate patients and families on disease processes, treatment options, and end-of-life care.
- Maintain accurate documentation of patient care activities and progress.
- Participate in case management meetings to discuss patient status and adjust care plans accordingly.
- Ensure compliance with medical terminology standards and sterile processing practices.
Requirements
- Current nursing license in the state of practice.
- Strong knowledge of anatomy and medical terminology.
- Experience in hospice or palliative care preferred but not required.
- Ability to perform catheterization, tube feeding, phlebotomy, and spinal taps is a plus.
- Familiarity with memory care techniques and behavioral health principles is beneficial.
- Excellent communication skills with a compassionate approach to patient care.
- Strong organizational skills with attention to detail in documentation and reporting.
- Willingness to work collaboratively within an interdisciplinary team environment.
