Job Description:
The RN Case Manager in Home Health provides skilled nursing care in patients' homes and serves as the primary care coordinator. This role allows for developing long-term relationships with patients and their families while working with an interdisciplinary team to deliver comprehensive care.
Responsibilities:
- Patient Care:
- Develop and implement individualized care plans in collaboration with patients, their families, and their physicians.
- Teach and assist patients and caregivers in self-care techniques, including medication management, dietary guidance, and other care needs.
- Perform evaluations and provide treatment based on established goals and patient needs.
- Care Coordination:
- Coordinate and communicate care plans across interdisciplinary teams during case conferences.
- Maintain ongoing assessment of clinical, functional, and emotional status to ensure care goals are met.
- Ensure timely communication of significant changes in patients' conditions to physicians and team members.
- Documentation and Compliance:
- Document patient evaluations, goals, and progress accurately and comprehensively.
- Periodically review and revise care plans to meet evolving patient needs.
- Provide thorough discharge planning and documentation for transitioning care.
- Community and Education:
- Act as an integral part of the community by providing health counseling and education to patients and caregivers.
- Serve as a resource for health promotion and disease prevention within the home care setting.
Minimum Qualifications:
- Licensure:
- Active Registered Nurse (RN) license in Vermont and New Hampshire or a compact license.
- Education:
- Graduate of an accredited nursing program.
