Job Title:
Registered Nurse (RN) – Case Manager
Location:
Bozeman, MT
Contract Details:
- Duration: 13 weeks
- Hours: 40 hours/week
- Shift: Day Shift (7:30 AM - 4:30 PM), with weekend rotations
Position Overview:
The RN Case Manager collaborates with physicians, patients, and care teams to create and implement individualized care plans. Responsible for assessing needs, coordinating care, resolving barriers, and managing discharge planning to ensure seamless transitions and optimal patient outcomes.
Key Responsibilities:
- Conduct patient assessments and determine case management needs.
- Develop and implement personalized care plans with input from patients and care teams.
- Monitor, document, and adjust care plans to meet patient goals.
- Coordinate behavioral health and long-term care services for continuity of care.
- Facilitate discharge planning for smooth transitions to appropriate settings.
Qualifications:
Required:
- Active Montana RN license
- American Heart Association BLS certification
- 2 years of nursing experience
Preferred:
- Bachelor’s Degree in Nursing
- 2 years of acute care case management experience
Skills:
- Strong judgment, communication, and teamwork skills
- Proficient in EMR systems and MS Office
- Ability to prioritize and perform in a dynamic environment
Physical Requirements:
- Frequent standing, walking, and lifting (up to 50 lbs)
- On-call availability and flexibility for weekends
