Job Description: RN Home Health/Hospice – Socorro, NM
Position Overview:
We are seeking a dedicated and experienced Home Health/Hospice Registered Nurse (RN) for a 13-week contract in Socorro, NM. This role requires a compassionate and flexible team player who thrives in a fast-paced environment, with a focus on providing exceptional care to patients in their homes.
Contract Details:
- Duration: 13 weeks
- Hours: 36 guaranteed hours per week
- Shift: 8-hour varied shifts starting at 8 AM (shifts may fluctuate to meet departmental needs).
- Includes one 8-hour on-call shift per 6 weeks.
- Weekday, rotating weekends, and holiday coverage required.
- Start Date: TBD
Key Responsibilities:
- Perform approximately 5-6 home visits per day, delivering comprehensive and patient-centered nursing care.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Administer medications, manage wound care, and provide health education to patients and families.
- Utilize EPIC EMR for documentation and care coordination (training provided during orientation).
- Adhere to agency policies, state regulations, and standards of care for home health and hospice services.
Required Qualifications and Certifications:
- Experience:
- Minimum of 2 years of current experience in Home Health/Hospice or related specialty.
- Must have completed at least one 13-week travel assignment.
- Licenses:
- Active New Mexico RN license or Compact License with multistate privileges.
- Certifications:
- Current BLS certification through American Heart Association, American Health and Safety, or American Red Cross.
- Other Requirements:
- EPIC EMR experience preferred.
- Flexible, team-oriented, and accustomed to a fast-paced healthcare environment.
- Must disclose all time-off requests with the initial submission.
Additional Information:
- 75-mile straight-line radius rule: No local travelers will be considered.
- Candidates must not contact the facility directly and should be fully aware of their submission.
- Submission must include:
- Two professional references (one from a current manager).
- Updated resume.
- Completed skills checklist/competency assessment.
Orientation:
- Orientation will take place during the first week and will not exceed 40 hours, including EPIC training.
