Job Summary:

Key Responsibilities:
- Assess patients’ health status, including physical, emotional, and psychosocial needs, in their home environment.
- Develop, implement, and regularly update individualized care plans in collaboration with the healthcare team.
- Administer medications, treatments, and procedures as prescribed by the physician.
- Monitor patient progress and document care provided, ensuring compliance with regulatory and agency standards.
- Educate patients and families about their diagnosis, treatment plan, medication usage, and self-care techniques.
- Coordinate with other healthcare professionals (e.g., therapists, social workers) to provide comprehensive care.
- Provide emergency care as necessary and respond to changes in patients’ conditions promptly.
- Maintain up-to-date and accurate patient records using electronic medical record (EMR) systems.
- Ensure compliance with infection control and safety protocols while delivering care.
- Participate in staff meetings, in-service training, and quality improvement initiatives.
Required Certifications and Licensure:
- Registered Nurse (RN) License: Active, unencumbered license in the state of practice.
- Basic Life Support (BLS): Certification through the American Heart Association or equivalent.
- Advanced Cardiovascular Life Support (ACLS): Preferred for more acute patient cases.
- Home Health-Specific Certifications (optional but beneficial):
- Certified Home Health Nurse (CHHN)
- Certified Hospice and Palliative Nurse (CHPN)
- Wound Care Certification (e.g., WCC, CWOCN)
- Driver's License: A valid driver’s license with proof of insurance, as the role involves travel to patient homes.
