Job Summary:
The Home Health RN provides skilled nursing care to patients in their homes, ensuring they achieve the highest possible level of independence and well-being. This role involves collaboration with physicians, patients, families, and the healthcare team to create and implement individualized care plans. The RN also educates patients and families about managing their conditions and maintaining optimal health.

Key Responsibilities:

• Assess patients’ health status, including physical, emotional, and psychosocial needs, in their home environment.
• Develop, implement, and regularly update individualized care plans in collaboration with the healthcare team.
• Administer medications, treatments, and procedures as prescribed by the physician.
• Monitor patient progress and document care provided, ensuring compliance with regulatory and agency standards.
• Educate patients and families about their diagnosis, treatment plan, medication usage, and self-care techniques.
• Coordinate with other healthcare professionals (e.g., therapists, social workers) to provide comprehensive care.
• Provide emergency care as necessary and respond to changes in patients’ conditions promptly.
• Maintain up-to-date and accurate patient records using electronic medical record (EMR) systems.
• Ensure compliance with infection control and safety protocols while delivering care.
• Participate in staff meetings, in-service training, and quality improvement initiatives.

Required Certifications and Licensure:

• Registered Nurse (RN) License: Active, unencumbered license in the state of practice.
• Basic Life Support (BLS): Certification through the American Heart Association or equivalent.
• Advanced Cardiovascular Life Support (ACLS): Preferred for more acute patient cases.
• Home Health-Specific Certifications (optional but beneficial):
• Certified Home Health Nurse (CHHN)
• Certified Hospice and Palliative Nurse (CHPN)
• Wound Care Certification (e.g., WCC, CWOCN)
• Driver's License: A valid driver’s license with proof of insurance, as the role involves travel to patient homes.

Preferred Qualifications:

• Minimum of 1-2 years of nursing experience, preferably in home health or community health.
• Proficiency in EMR systems and documentation.
• Strong communication, problem-solving, and time-management skills.
• Ability to work independently and adapt to various home environments.
