Job Title: UR Case Manager/Care Coordinator (RN)

Location: Espanola, NM
Contract Duration: 13 Weeks
Guaranteed Hours: 40 Hours per Week
Shift: 8-Hour Varied Shifts

Position Overview:

We are seeking an experienced Utilization Review (UR) Case Manager/Care Coordinator RN for a 13-week contract in Espanola, NM. The ideal candidate will have a strong background in inpatient/acute care utilization review and case management, ensuring appropriate patient care coordination and resource utilization. This role requires adaptability in a fast-paced environment and a commitment to quality patient care.

Please note: This position follows a 75-mile straight-line radius rule, meaning local travelers are not eligible. Candidates must be aware of their submission to the facility.

Key Responsibilities:

  • Perform utilization review and case management for inpatient and acute care patients.
  • Assess patient needs and coordinate care to ensure appropriate utilization of resources.
  • Collaborate with interdisciplinary teams to develop and implement effective care plans.
  • Ensure compliance with hospital policies, regulatory guidelines, and payer requirements.
  • Monitor patient progress, document reviews, and communicate findings to healthcare providers.
  • Float to other areas within the facility or within a 20-mile radius as needed.

Qualifications & Requirements:

  • Experience:
  • Minimum 12 months of recent inpatient/acute care UR experience required.
  • At least one previous 13-week travel assignment preferred.
  • Minimum two years of specialty experience required.
  • Certifications:
  • BLS certification (Must be from AHA, American Health and Safety, or American Red Cross).
  • Technical Skills:
  • EPIC EMR experience required.
  • Xsolis experience preferred.
  • Additional Requirements:
  • Must be a team player, flexible, and able to work in a fast-paced environment.
  • Must be willing to float within the facility and alternate locations within a 20-mile radius.
  • Candidates must provide two professional references (one from a current manager), a resume, and a completed skills checklist/competency assessment.

Shift & Schedule:

  • Varied 8-hour shifts, starting at 8:00 AM.
  • May fluctuate to meet department staffing needs.
  • Includes weekday shifts, with rotating weekends and holidays as needed.
  • Traveling pairs are not guaranteed the same schedule or shifts.

Additional Information:

  • Orientation: First week of the contract (not exceeding 40 hours), including EPIC training.
  • Scrubs: Required but not provided by the facility.
