Job Title: UR Case Manager/Care Coordinator (RN)

Position Overview:
We are seeking an experienced Utilization Review (UR) Case Manager/Care Coordinator RN for a 13-week contract in Espanola, NM. The ideal candidate will have a strong background in inpatient/acute care utilization review and case management, ensuring appropriate patient care coordination and resource utilization. This role requires adaptability in a fast-paced environment and a commitment to quality patient care.
Please note: This position follows a 75-mile straight-line radius rule, meaning local travelers are not eligible. Candidates must be aware of their submission to the facility.
Key Responsibilities:
- Perform utilization review and case management for inpatient and acute care patients.
- Assess patient needs and coordinate care to ensure appropriate utilization of resources.
- Collaborate with interdisciplinary teams to develop and implement effective care plans.
- Ensure compliance with hospital policies, regulatory guidelines, and payer requirements.
- Monitor patient progress, document reviews, and communicate findings to healthcare providers.
- Float to other areas within the facility or within a 20-mile radius as needed.
Qualifications & Requirements:
- Experience:
- Minimum 12 months of recent inpatient/acute care UR experience required.
- At least one previous 13-week travel assignment preferred.
- Minimum two years of specialty experience required.
- Certifications:
- BLS certification (Must be from AHA, American Health and Safety, or American Red Cross).
- Technical Skills:
- EPIC EMR experience required.
- Xsolis experience preferred.
- Additional Requirements:
- Must be a team player, flexible, and able to work in a fast-paced environment.
- Must be willing to float within the facility and alternate locations within a 20-mile radius.
- Candidates must provide two professional references (one from a current manager), a resume, and a completed skills checklist/competency assessment.
Shift & Schedule:
- Varied 8-hour shifts, starting at 8:00 AM.
- May fluctuate to meet department staffing needs.
- Includes weekday shifts, with rotating weekends and holidays as needed.
- Traveling pairs are not guaranteed the same schedule or shifts.
Additional Information:
- Orientation: First week of the contract (not exceeding 40 hours), including EPIC training.
- Scrubs: Required but not provided by the facility.
