What are the responsibilities and job description for the Newborn Coordinator position at Swope Health Services?
Under pop health for assigned to children’s services and clinic task, training , function as directed by the CHO, EVPCS or Designated Pediatric Medical Director. Works with clinic managers to ensure scheduling and patient management occurs appropriately under clinic workflow. The purpose of the Newborn Coordinator is to increase access to care and facilitate the management of newborns (birth – 2 months of age) entering the Swope Health System. The Newborn Coordinator shall coordinate newborn management across all services sites to facilitate the transition of care from the nursery or NICU to primary care by a Swope Health provider to ensure the delivery of safe, consistent, quality care. This associate also provides a variety of direct services to Swope Health patients designed to promote health, identify and address risks, and support the maintenance of a safe nurturing environment.
Primary Functions:
- Facilitate the Transition of Care from Nursery/NICU to Primary Care (Newborn Transition of Care) for all swope health sites.
- Pre- and post-Visit Coordination of Care
- Newborn Care may include collecting blood samples and OAE screening.
- Newborn Monitoring and Tracking from Nursery, NICU, Hospital etc
- Pediatric/OB Integration
- Duties as assigned
Educational Requirements
MA preferred, Education in social work, nursing, case management, or related field; Associate’s degree, Certification as Medical Assistant or higher accepted.
Experience & Qualifications
- Patience and ability to establish rapport with the assigned population
- Flexible schedule and transportation
- Knowledge about diverse clientele
- Ability to write clearly and distinctly
- Knowledgeable about safe sleep resources
- Must have a current, valid driver’s license, transportation and appropriate automobile insurance and be able to drive extensively
- Knowledge in use of electronic health records, Microsoft Office applications, and virtual technologies including video conferencing.
