What are the responsibilities and job description for the Qualified Mental Health Professional position at Swope Health Services?
The Qualified Mental Health Professional is responsible for completing holistic psychosocial assessments and treatment planning, community support, and crisis intervention services in the office, client homes, and in the community for children and adults with serious emotional disturbances, serious mental illness, and substance use disorders.
Successful Candidate will have :
- Facilitate an assessment interview that produces sufficient information to identify a BH and or SUD diagnosis, if such is present.
- Assist in the overall implementation of the CPR program and in its continual growth and development.
- Ensure timely completion of initial and annual psycho-social assessments for children, youth and adults participating in the CPR program that integrate physical and mental health concerns and identify strengths and functional challenges using the Daily Living Assessment.
- Develop specific, measurable, and person centered treatment recommendations and Care Plans in partnership with the treatment team to guide and develop services with individuals served.
- Participate in interdisciplinary treatment team meetings to provide clinical support and consultation to team members toward the advancement of consumer treatment plan goals and holistic wellness initiatives.
- Provide community support interventions to consumers and families within SHS and the community as needed and indicated by the individual's Care Plan.
- Participate in community stakeholder meetings related to the DFS Level IV, CASSP and System of Care Planning meetings as needed.
- Attend and participate in department meetings as appropriate.
- Participate in Performance Improvement activities related SHS Quality Performance Initiatives.
- Maintain productivity standards as established by the organization.
- Other duties as assigned.
Education / Qualification
