What are the responsibilities and job description for the Integrated Behavioral Health Clinician position at Swope Health?
Responsible for the provision of counseling and case management services for clients and their families providing comprehensive care and collaborative treatment planning on site during office hours. In cooperation with the assigned schools, this position provides clinical behavioral health services and related support to students and their families.
Successful Candidate will have:
- Perform intake assessments, which include the completion of the initial intake assessment, 6 month reviews and annuals as needed, to address specific needs.
- Completion of chart documentation reviews that include the Psychosocial History, Treatment Planning, Chart Progress Notes, Quarterly Reviews, and Discharge Summaries within established guidelines and time frames.
- Engages individuals and families as partners in services and develops positive, hopeful, and trusting relationships.
- Monitors, evaluates, and records client progress with respect to treatment goals and modifies treatment plans according to changes in client status.
- Perform individual, family and group counseling utilizing approved evidence-based treatment approaches as sanctioned by the Swope Behavioral Health Department.
- Provide brief-targeted case management services to assist with coordinating support and connection to community resources for holistic care.
- Evaluates crisis situations and applies appropriate interventions.
- Actively participates in interdisciplinary treatment team meetings that support an integrated health care model to provide comprehensive care for clients.
- Accurately report billing documentation consistent with the requirements of the payer or contract, i.e. Missouri Department of Mental Health, Missouri Department of Social Services, private insurance, Manage Care contract, Mental Health Levy, etc.
- Meet expected productivity guidelines and standards.
- Participate in Continuous Performance Improvement activities.
- Must possess and demonstrate expertise around diagnostic formulation as it relates to the most current DSM.
- Perform other duties as assigned.
Education/Qualifications
- Master’s degree in Social Work, Psychology, Counseling and Guidance or related field.
- Must be licensed (Provisional or Clinical) to provide mental health services in the State of Kansas
- Provisionally Licensed Master Social Worker (LMSW) under approved supervision, Provisional Licensed Professional Counselor (PLPC) under approved supervision, or other provisionally licensed mental health professional under approved supervision
- Preferred Licensed as Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT).
