What are the responsibilities and job description for the POPULATION HEALTH NURSE position at Swope Health?
As the Population Health Nurse for Swope Health, you will be part of a team or caregivers that provide care for those who need it the most. The population health nurse is responsible for promoting population health by applying the nursing process with individuals, families, and communities. The Population Health nurse communicates with patients to update on lab testing results and communicates any abnormalities and advises of results and further care options. Primary Responsibilities :
- Transcribes physician / provider orders, collect specimens and cultures for laboratory analysis.
- Reviews telephone encounters and addresses within the LPN scope of practice.
- Completes forms and documents and obtains physician signature as appropriate. This includes but not limited to diabetic supply form, DME, and Home Health Orders.
- Arranges referrals for specialty care, urgent or emergent care at the direction of the provider. Instructs patients in home self-care when an immediate clinic evaluation is not necessary. Instruct patients to come to the clinic as necessary. Consult with the clinic manager or provider when there is a question about the severity of the problem or the need for immediate attention.
- Administers oral and parenteral (IM, subcutaneous, intradermal) medications as ordered and prescribed by the provider using the 6 Rights of Medication Administration. May dispense non-prescription sample medications kept in the clinic only. May not dispense prescription sample medications.
- Performs waive testing and venipuncture as ordered by the provider.
- Documents all nursing care and treatments performed, in the electronic medical record.
Coordinates pre-visit planning by reviewing the electronic medical record to include patient history and proactively orders labs and other testing based on standing orders.
