What are the responsibilities and job description for the Home Care Coordinator | PACEKC position at Swope Heath?
As a Home Care Coordinator for PACEKC, you will be part of a team of caregivers to provide care for the people who need it most. Our Home Care Coordinator oversees the day-to-day operation of the Home Care Program to ensure compliance and quality. While ensuring the optimal level of independence for our participants. As a member of interdisciplinary team, you will work closely with the clinical staff to establish a home care plan of care to meet participants' needs. In this role you will:
- Completes initial nursing and home care assessments for all new PACE participants in their home environment to determine their individual needs.
- Coordinates closely with the clinical team for ongoing, semi-annual participant assessment to develop and maintain an up-to-date and accurate home care Plan of Care that is responsive to changing participant needs. As needed, performs change of status and/or semi-annual participant assessments for complex cases.
- Assures that home care associates provide skilled supportive and restorative nursing care to participants as ordered, based on the plan of care, participant needs and document timely and appropriately in the medical record.
- Assure timely education and training for participants, caregivers, and families on an individual or group basis as needed.
- Responsible for the coordination of personal care activities of all participants in the home.
- Manage and prioritize utilization of homecare services provided and fiscal responsibility for, departmental budgetary requirements.
- Participates in the after-hours and weekend on-call rotation and coordinates 24-hour care delivery.
- Maintains up-to-date and appropriate documentation in the participant's electronic medical record.
- Hires, selects, directs, supervises, and evaluates the performance of direct reports. Handles discipline and termination, if necessary, of associates in accordance with policy.
- Oversees the daily workflow of the department.
- Assists in providing in-service education for the PACE KC associates.
Successful Candidates will have:
- Bachelor of Science in nursing (BSN) or equivalent experience.
- Active state licensed Registered Nurse (RN).
- (3) years nursing management, preferably in home and community health.
- Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.
- Current driver's license, auto insurance, and dependable transportation.
- experience working with the elderly population.
- Current American Heart Association Basic Life Support (BLS) with Automated External Defibrillator (AED) certificate or the ability to obtain within two weeks of hire, before having direct participant contact.
