Summary (of position):

Swords to Plowshares Service Center currently provides community service programming for senior veterans and adult veterans with disabilities. Current services provided are as follows: 

The Licensed Vocational Nurse/ Licensed Psychiatric Technician (LVN/LPT) is responsible for the delivery and coordination of medical and behavioral health case management services offered at our Veteran Community Center. The LVN/LPT at the Veteran Community Center plays a critical role in providing healthcare support to veterans in a community-based setting. Additionally, the LVN/LPT is responsible for delivering direct nursing care, assisting with case management, and collaborating with a multidisciplinary team to support the health and well-being of veteran clients.

 

Responsibilities:

  • Provides case management services related to medical and behavioral health 
  • Coordination of care with community medical providers 
  • Assessment and assistance with veteran ADL skills  
  • Assessment and referrals to local clinics 
  • Assisting veterans in obtaining public benefits (i.e Cal Fresh, Medi-Cal, Medicare,etc.) 
  • De-escalation and crisis intervention support 
  • Facilitates weekly wellness groups 
  • Timely data entry and record management 
  • Attend all mandatory training and meetings 
  • Provides non-emergency medical support as needed 
  • Ensures compliance with federal and state regulations for the delivery of community behavioral healthcare services.  
  • Performs other duties as assigned by Program Manager and Clinical Director of Non-Residential Services. 

Desired Qualifications:

Education:

  • CA LVN or LPT license- current and in good standing
  • CPR/First Aid certification or willingness to obtain within 60 days of employment

Experience:

  • At least 1 year of experience working in a community behavioral healthcare setting. 
  • Experience with populations impacted by mental health, substance dependency, and homelessness.

Compensation is based on a competitive public interest salary scale. Swords to Plowshares offers a generous benefits package:

Please note: Any employee who is on-call, in a temporary position, and/or less than 50% FTE status is not eligible for benefits aside from the company sponsored EAP, paid lunch hour, and supportive environment. Vacation, sick and time off are pro-rated based upon FTE percentage.
