Job Summary :
- Assist persons with disabilities in pursuing their personal futures and goals through residential supports, volunteer opportunities, and community interaction.
- Assist with planning, coordinating supports, providing direct supports, and developing and maintaining relationships.
- Monitor support plans.
- Provide flexible assistance as the needs of the individual changes.
- Assist and support people with disabilities in gaining citizenship, involvement in their communities, and the development of friends and services outside the human service system.
