The Engagement Specialist will be responsible for engaging and educating patients and their caregivers on the benefits of the Sylvan nutrition program. This individual plays a key role in developing, growing, and sustaining the Sylvan Health program.
This is a full-time, remote position reporting to the Engagement Manager.

Job Description
- Be the patient/caregiver's first point of contact with the Sylvan Health team
- Schedule telehealth consultations and follow-up visits with one of our experienced dietitians
- Develop a relationship with a patient and/or their caregiver quickly to build and maintain trust
- Demonstrate empathy upon each patient or caregiver interaction
- Make outbound calls to patients and/or their caregivers to educate about Sylvan Health and enroll them in the program
- Communicate and collaborate effectively with dietitians and other care team members daily
- Document patient information in an Electronic Medical Record
- Resolve patient and caregiver questions with timeliness and professionalism
- Work across different systems and software
- Send communication to patients and caregivers via email, telephone calls, text messages, mail, and Electronic Medical Records
- High School Diploma or GED required
- Bachelor's Degree (preferred) or 3-5 years of comparable professional experience, ideally in a healthcare community outreach, navigation, engagement, or direct-to-consumer telephonic sales setting
- Familiarity with Electronic Medical Records
- Ability to communicate with clinicians
- Capacity to conceptualize workflow, develop plans, and implement appropriate actions
- Ability to competently use Microsoft Office, including Word, PowerPoint, and Excel
- Ability to take initiative and solve problems quickly
- Excellent interpersonal and communication skills
- Ability to work in a fast-paced, constantly evolving environment with a positive attitude
- Background in healthcare, social work, or public health preferred
